All-on-4 replaces an entire arch of missing teeth using just four dental implants. The result is a fixed, non-removable set of teeth that looks, feels, and functions like natural teeth.
It is designed for patients who are missing most or all of their teeth and want a permanent solution that does not require removing their teeth at night like traditional dentures. In most cases, bone grafting is not needed — the four implants are placed to work with the bone you already have.

Who Is a Good Candidate for All-on-4?
The “4” in All-on-4 refers to four implant posts. Two are placed straight toward the front of the jaw. Two are placed at an angle toward the back.
The angled rear implants are what make All-on-4 different. By tilting them, we can reach denser bone further back in the jaw that is often available even in patients with significant bone loss. This is why All-on-4 usually does not require bone grafting before placement.
All four posts are placed in the same surgical appointment. A provisional set of teeth is attached the same day or very shortly after. Your final permanent arch is placed once full healing is complete, typically 3-6 months later.
- Adequate bone density: The All-on-4 procedure uses angled implants that maximize the use of available bone. Therefore, you must have sufficient jaw bone to support the implants.
- Healthy gums:You must be free from any active periodontal disease or gum disease.
- Good overall health: You must have your chronic conditions under control, such as diabetes.
Patients with severe bone loss may still need grafting even for All-on-4. We determine this at your evaluation with imaging. The advantage of All-on-4 is that it expands access to implants for patients who might not otherwise qualify.
All-on-4 is not for patients who only need to replace a few teeth. If you are missing one to several teeth, single implants or a smaller implant-supported bridge may be a better fit.
All-on-4 vs. Traditional Dentures
Stability. All-on-4 is fixed to your implants. It does not move when you eat, speak, or laugh. Traditional dentures can slip, causing difficulty chewing and speaking.
Bone preservation. Dental implants stimulate the jawbone, which prevents the bone shrinkage that occurs after tooth loss. Traditional dentures sit on top of the gum and do not prevent bone loss. Over time, bone loss changes the shape of your face and causes dentures to fit less well.
Maintenance. All-on-4 is cleaned like natural teeth — brushing and flossing, plus regular professional maintenance visits. Dentures are removed for soaking and cleaning.
Longevity. All-on-4 implant posts can last a lifetime. Traditional dentures typically need relines every 2-3 years and replacement every 7-10 years.
All-on-4 vs. Traditional Full-Arch Implants
Traditional full-arch implant restoration uses 6-8 individual implant posts placed throughout the jaw. This approach typically requires extensive bone grafting in patients with bone loss, adds time and cost, and extends the treatment timeline significantly.
All-on-4’s four-implant design avoids much of this. It works with existing bone using angled placement and typically reduces the treatment timeline and cost compared to traditional multi-implant full-arch restoration. Both are valid options. The right choice depends on your bone density, jaw anatomy, and the long-term restoration plan. We discuss both at your consultation.
